* Discounts will be allowed as follows: $15.00 for two children from the same family, $25.00 for three children from the same family, no charge for any remaining children in the same family. The highest applicable fee listed above will be used in calculating discounts.

** A late fee of $20 will be added to the above fees for registrations received after 3/14/2013 for the Mustang, Bronco, and Babe Ruth 13 Divisions.

*** The parent or legal guardian that registers a player is expected to sell or purchase a minimum of 50 raffle tickets at $1.00 each. Two players from the same family will receive 70 tickets and three or more players from the same family will receive 90 tickets.

Items required for registration:

1)    A copy of each child's birth or baptism certificate, (new players only, please do not bring your original certificate, we can not make copies).
2)    Parents insurance company name and policy number.
3)    Each player should be present for uniform sizing.
4)    Check made payable to: Junior Major League of Milford. Check should be for the total number of players and if you choose to take the raffle buy out option.
